If you take a single hit greater than your CON score, you have sustained a major wound. You immediately roll 1d6 to determine the specific attribute you loose if any. You can then make a 1d20<= [your current hit points] to remain conscious. If you do remain conscious, you can attempt to take actions, but run the risk of worsening your condition.
Your Chirurgery Needed checkbox is immediately checked. You are considered unhealthy, and cannot effectively care for yourself. You must receive the care of a physician quickly. Once you are under the care of a physician, that doctor may make a chirurgery roll once per week to see if they can stop your condition from worsening. You must remain under care until at least half your hit points have returned, or risk deteriorating again.
While you are unhealthy, any activity beyond bedrest had a chance to worsen your condition, cause infection, etc.
So, Arne must enter the care of a physician before the week is out. If he does anything other than rest, his condition may worsen. He may, in fact, be unconscious for weeks, depending on his rolls. Travel is not often restful in this day and age. Trying to get him home could be dangerous to his health.
